It can kill honest feedback. As a leader, your reputation is extremely important. Optimism Bias causes us to paint ourselves in a more positive light than we are. Leaders are very susceptible because they are surrounded by those who seek to gain acceptance and favor. Nobody wants the boss to think they are not competent, so others shield the leader from reality. We as leaders, then pat ourselves on the back for doing good work.
